This is the JSNA chapter on cardiovascular disease. Theme chapters summarises implications for commissioning, who is at risk and why, the level of need in the population, service provision and use, unmet needs, what works in terms of evidence, community views and priorities, any related equality impact assessments, unmet service needs/gaps and recommendations for further needs assessment work. 

Profiles for each clinical commissioning group (CCG) and local authority summarising data about high blood pressure

The hypertension profiles complement Public Health England’s Tackling high blood pressure: from evidence to action report. Each PDF profile has chapters on prevention, detection and management. They have a wide range of data about hypertension for each local authority and clinical commissioning group (CCG) in England. They are for commissioners and health professionals to use when assessing the impact of hypertension on their local population, opportunities for improvement and making decisions about services and emerging issues.

This document shows progress against the actions set out in Living Well for Longer: National support for local action which followed a call to action from the Secretary of State. bThe report shows that there has been improved prevention, early diagnosis and treatment of the 5 big killers:

  • cancer
  • stroke
  • heart disease
  • lung disease
  • liver disease

The packs identify potential opportunities for improving outcomes, quality and efficiency at clinical commissioning group (CCG) level. CCGs can use the packs to improve the value of their commissioned CVD services by identifying where to look across the pathway which patients take through the health system, and what to change.
